Anthropic's public API has no /health endpoint. Decide:

Option A: do not bind /health at all (clients won't expect it; do not advertise it).
Option B: bind /health only when extra.expose_health: true (escape hatch for k8s probes / dashboards).

Default to Option A. Implement B as opt-in. Do NOT mount /health/detailed — that is an api_server-specific extension with no Anthropic equivalent.
